1099 Forms

PERSI will begin mailing 1099 forms to retirees on or before January 26, 2009; the mailing will be completed no later than January 31, 2009. The 1099 forms for Retirees will also be available for download from the PERSI Web site beginning Friday, January 16, 2009. To download your 1099, click on Account Information under the Retiree dropdown menu on the left side of the PERSI homepage; scroll down and click on the myPERSI link, log on to your account using your email address and Password, then follow the instructions.
